Restaurant Meals Program (RMP) Intake Form
Welcome to the CalFresh Restaurant Meals Program (RMP) Intake Form!
The RMP is a program that allows eligible CalFresh households to use their Electronic Benefits Transfer (EBT) card to purchase prepared meals at participating restaurants. This includes households comprised of only the following members:
- Adults aged 60 or older (and their spouse);
- People with a disability (and their spouse); or
- Individuals experiencing homelessness.
How It Works:
Your restaurant’s participation in the RMP requires approval from both the California Department of Social Services (CDSS) and the United States Department of Agriculture - Food and Nutrition Service (USDA-FNS). Once you submit this Intake Form, we at CDSS will review your submission. If approved, we will email the restaurant owner an RMP contract to sign; if denied, we will notify you by email. Once you have a signed RMP contract, you will then use that contract to apply with USDA-FNS, per instructions at http://www.cdss.ca.gov/RMP/partners.
Important Instructions Before You Begin:
- Complete these forms on ONE computer in ONE session.
- Do not use your browser’s back button or navigate away from the Intake Form during your session.
- Have all your restaurant information ready before completing this form, including:
- Restaurant name(s) and address(es)
- Discounted pricing details for RMP customers
- Healthy meal options offered
